Hello class, my name is Rachel and I am nearing (finally) the end of my MHA degree. I have been a nurse for over 15 years and decided to transition from clinical healthcare to administrative because I have worked in many organizations where administrative leaders have little to know clinical knowledge, yet make substantial decisions impacting clinical workflow and patient care. That being said, I have worked over 10 years in the federal healthcare system as a military spouse and recently relocated to Japan after living in Germany for 6.5 years. I actually am writing this in quarantine (thanks Covid-19) as military regulations require us to quarantine for 14 days after arriving in the country. My career goals have been modified quite a bit in the last year, as I was gearing up to move back to the US and have a lot more employment opportunities, but like always, the military had different plans for us. My employment options are very limited here in Japan but I hope to make the best out of the situation and hopefully can find some employment opportunities to enhance my professional development.
As I mentioned, one of the main reasons I switched from clinical to administrative roles is because I felt decision makers needed clinical guidance and knowledge. When thinking about the role of interest groups in policy-making, I think it’s fair to say a wide variety of stakeholders need to be actively engaged in the policy process to best suit all stakeholders. I say this because a policy can improve things for one group of stakeholders while providing negative effects for another group. For example, access to care has been at the forefront of many healthcare issues; however, shortening appointment times to allow for more appointment slots will improve access to care for patients but could decrease the quality of care provided from providers and increase the workload of those clinicians. I can speak from experience that this change, while intended to have positive impacts on patient care actually leads to increased frustration from both providers and patients because patients are rushed out of their appointments feeling like the provider isn’t listening. That being said, focus groups for policy making should include a variety of stakeholders such as providers, patients, public figures, insurance agents and government officials if available to create an environment to discuss policies that will affect positives and negatives for each stakeholder group to best approach local and national levels. Furthermore, it is important to remember that many healthcare policies and decisions are made at the state level and don’t require fe.
Consensus statement

1) The right to associate and organize. 2) The right to be protected by law. 3) The right to be free from violence. 4) The right to be free from discrimination. Without these incentives, behavioral health providers have limited ability to track health outcomes, engage in prevention efforts, and ensure continuity of care between primary care and behavioral health services. Hello class, my name is Rachel and I am nearing (finally) the end of my MHA degree. I have been a nurse for over 15 years and decided to transition from clinical healthcare to administrative because I have worked in many organizations where administrative leaders have little to know clinical knowledge, yet make substantial decisions impacting clinical workflow and patient care. That being said, I have worked over 10 years in the federal healthcare system as a military spouse and recently relocated to Japan after living in Germany for 6.5 years. I actually am writing this in quarantine (thanks Covid-19) as military regulations require us to quarantine for 14 days after arriving in the country. My career goals have been modified quite a bit in the last year, as I was gearing up to move back to the US and have a lot more employment opportunities, but like always, the military had different plans for us. My employment options are very limited here in Japan but I hope to make the best out of the situation and hopefully can find some employment opportunities to enhance my professional development.
As I mentioned, one of the main reasons I switched from clinical to administrative roles is because I felt decision makers needed clinical guidance and knowledge. When thinking about the role of interest groups in policy-making, I think it’s fair to say a wide variety of stakeholders need to be actively engaged in the policy process to best suit all stakeholders. I say this because a policy can improve things for one group of stakeholders while providing negative effects for another group. For example, access to care has been at the forefront of many healthcare issues; however, shortening appointment times to allow for more appointment slots will improve access to care for patients but could decrease the quality of care provided from providers and increase the workload of those clinicians. I can speak from experience that this change, while intended to have positive impacts on patient care actually leads to increased frustration from both providers and patients because patients are rushed out of their appointments feeling like the provider isn’t listening. That being said, focus groups for policy making should include a variety of stakeholders such as providers, patients, public figures, insurance agents and government officials if available to create an environment to discuss policies that will affect positives and negatives for each stakeholder group to best approach local and national levels. Furthermore, it is important to remember that many healthcare policies and decisions are made at the state level and don’t require fe.

How This Course & Content Have Real-Word Application
We are witness to history and are living in one of the most active times in our country’s history for healthcare
reform. In 1966, the Medicare Act was signed into law by President Johnson, the most significant piece of
healthcare legislation in our country to that point. Fast forward from 1966 to 2010 and the passing of the
Affordable Care Act, which arguably is the second most impactful piece of legislation on U.S. health care
since the Medicare Act.
Medicare has grown significantly since 1966 and is now about 14% of our national budget, covering 47 million
Americans (Kaiser Family Foundation, 2015). Government health plans (Medicare, Medicaid, Tri-Care,
Veteran’s Administration) are growing and are on pace to insure more lives in the near future than lives

The Pervasive Influence of Religion on Social-Service Provision
While the Supreme Court has been busy changing the applications of the free-exercise and establishment clauses, the American legislature and past presidential administrations have enacted public policy that blurs the line between church and state. These developments have contributed to the growing debate involving public-private partnerships, specifically religious ones. Following the 1996 Welfare Reform act, which set up charitable-choice provisions that provided eligibility and protections for religious organizations, the government began contracting out funds for social services. Quickly, the government increased the provision of these services to private for-profit, non-profit, and faith-based organizations. These provisions, along with the legal and legislative developments, have accommodated these faith-based groups by increasing their direct federal funding and creating exemptions for them from generally applicable laws.
According to Harvard Law Professor Martha Minow, state and federal governments are increasingly exploring privatization. Privatization efforts involve aiding outside organizations in performing public responsibilities and addressing public needs. Governments do this, in part, by outsourcing, subsidizing, and directly funding the provision of social services to churches and religious, nonprofit and for-profit organizations (1232). These public resources are then used to promote the involvement of religious organizations in public welfare that often incorporate religious practices or discrimination (1233).
According to the Hoover Institution’s article, “Why ‘Faith-Based’ Is Here to Stay” author Lew Daly argues that the government’s increased support for the Bush-era “faith-based initiative” was not only inevitable but also highly beneficial due to shared goals between government and religious institutions. Daly argues, if faith-based programs prove to be making a positive difference then current legal and legislative barriers separating religious activity from federal provision of social services could be seen as discriminatory and counterproductive (46).
However, direct funding of social services provided by religious institutions raises several serious concerns. According to David Saperstein, a Georgetown Law professor, “there exist many constitutional, policy, and moral constraints on direct funding of religious organizations that would make the inclusion of these [public-religious] partnerships…impossible or inadvisable” (1353). There’s an obvious dilemma involved with the privatization and outsourcing of public funds and provision of services to pervasively religious organizations. For one, direct funding departs from a very common sense conception of the First Amendment, a mandate designed to separate religion and state, by creating clear limits on the government endorsem.

Throughout this course you will get a broad overview of the laws and regulations that govern human services disciplines. Some of you will be pursuing social services which are governed by county laws and regulations which typically stem from federal regulations. Some of you will be pursuing non-profit work which also has a different degree of governing and bylaws. We will explore how they govern and change from a state to state level and national level. In this first week, “It all begins with policy” you will gain an understanding of the foundation on which human services derives. Building upon this foundation is an understanding of the process of legislative and community advocacy: a process where human service and professional organizations engage in lobbying efforts to encourage action on current issues affecting the provision of services, consumer rights, or professional autonomy. Some of you may be able to relate it to work or agencies you are currently employed in or be able to understand how it relates to areas you wish to pursue once your degree is completed.
There is no written assignment for the first week (don’t worry, there will definitely be some coming!) so I encourage you to put critical thought and perspective into your discussion post and your replies to your classmates. Your discussion for this week will center on lobbying and advocacy which you will find is a major component in human services work.

The economic structure that each society has its laws, institutions, policies, etc. results in different distributions of economic benefits and burdens across members of the society. These economic frameworks are the result of human political processes and they continually change both across societies and within societies over time.
Autonomy- non influenced decisions for both patients and physicians
The term “autonomy” has appeared more and more often in the medical literature. According to this interpretation of autonomy, the goal for an autonomous person is to decide on his or her own, without undue manipulation by others.
One of the first empirical papers in medical decision making on patient autonomy thus linked autonomy to the question of whether patients wanted to make decisions themselves. In the descriptive medical decision making literature, this meaning has become the default.
The patient described in the informative model from the well known paper of Emanuel and Emanuel corresponds with an autonomous patient in this sense.

Gene therapy can be broadly defined as the transfer of genetic material to cure a disease or at least to improve the clinical status of a patient.
One of the basic concepts of gene therapy is to transform viruses into genetic shuttles, which will deliver the gene of interest into the target cells.
Safe methods have been devised to do this, using several viral and non-viral vectors.
In the future, this technique may allow doctors to treat a disorder by inserting a gene into a patient's cells instead of using drugs or surgery.
The biggest hurdle faced by medical research in gene therapy is the availability of effective gene-carrying vectors that meet all of the following criteria:
Protection of transgene or genetic cargo from degradative action of systemic and endonucleases,
Delivery of genetic material to the target site, i.e., either cell cytoplasm or nucleus,
Low potential of triggering unwanted immune responses or genotoxicity,
Economical and feasible availability for patients .
Viruses are naturally evolved vehicles that efficiently transfer their genes into host cells.
Choice of viral vector is dependent on gene transfer efficiency, capacity to carry foreign genes, toxicity, stability, immune responses towards viral antigens and potential viral recombination.
There are a wide variety of vectors used to deliver DNA or oligo nucleotides into mammalian cells, either in vitro or in vivo.
The most common vector system based on retroviruses, adenoviruses, herpes simplex viruses, adeno associated viruses.
